How do I decline and appointment?

It occurs from time to time, let me show you how to address it;  

While you are in the Dashboard go to Notifications. For appointments booked online, and confirmed by the client, you will see the option to OK or decline on the far right-hand side of the screen.  Keep in mind though once you see this notification, the appointment is already on your calendar and Okaying it just remove the notification from your dashboard.
